Sciatica is a common condition in which pain starts in the lower back or buttock and travels down the leg. It occurs when one of the nerves that contribute to the sciatic nerve becomes irritated or compressed.
What does sciatica feel like?
Sciatica can cause pain that travels from the lower back into the buttock, thigh, calf or foot. Some patients describe it as burning, shooting or electric-shock-like pain.
Other symptoms may include:
- Tingling or pins-and-needles sensation
- Numbness in the leg or foot
- Weakness in the leg
- Pain aggravated by sitting, bending, coughing or sneezing
- Difficulty walking or standing for prolonged periods
What causes sciatica?
Common causes include:
- Slipped or herniated disc
- Lumbar spinal canal stenosis
- Degenerative changes in the lumbar spine
- Spondylolisthesis
- Compression or irritation of a spinal nerve
Does sciatica always require surgery?
No. Many patients improve with appropriate non-surgical treatment. Depending on the cause and severity, treatment may include activity modification, medications, physiotherapy and guided exercises.
Surgery may be considered when there is persistent severe pain despite appropriate treatment, progressive weakness, significant nerve compression or certain neurological warning signs.
When should you consult a spine specialist?
Seek specialist evaluation if leg pain is persistent or worsening, particularly when associated with numbness, weakness or difficulty walking.
Urgent medical evaluation is required if you develop new loss of bladder or bowel control, numbness around the groin/saddle region, or rapidly worsening weakness.
